As a multidisciplinary team made up of nurse manager, district nurses, social workers, GPs, Northland Health inpatient services and Te Ha O Te Oranga, Kaipara Palliative Care works closely as a team to ensure care co-ordination and services are delivered in a timely co-ordinated way for the benefit of the patient and their families.
